Notifying Parents about Student Illness or Injury

Please report all suspected or diagnosed illnesses to the school by using this form, Absence/Tardy, or by calling the school (612-788-9095). The Health Office must be notified when a student has a contagious condition such as chicken pox, strep throat or head lice so that appropriate measures may be taken. Parent fact sheets will be emailed to families of sick children; they contain information about when a child may return to school.

If your child has an injury requiring any adaptive equipment such as crutches, braces, splints or a wheelchair, the school must have a note from a doctor explaining the necessity and the length of time the equipment will be needed. If your child is unable to participate in physical education or recess due to injury or illness, a doctor’s note is required and the Health Office must be notified. The Health Office will notify appropriate school staff members of the situation.
