School policy and state law require each volunteer to have a background check completed through the school. State Law does not allow public schools to pay for these background checks. Therefore, each individual must pay $20 for a one-time background check. These forms are available in the school office, or by clicking –>Here.  Please remember to either mail or return the completed form with check to the main office.

Processing of these forms takes approximately one week.
